New patient paperwork: please call us at (970) 221-1993 or (307) 432-3924 and we’ll either email or postal mail you the forms you’ll need to complete and bring to your first appointment.

What to bring to your appointment: In order for us to better serve you, please bring:
▪Current insurance card
▪Driver’s license or other form of photo ID
▪A copy of any reports, x-rays, discs, or films
▪A list of all your current medications (name, dosage, number of times taken per day)

Financial policy: Neurology Specialists of the Rockies is a provider for Medicare and most major insurance plans. As a courtesy, we provide insurance billing. Anything not covered by insurance will be your responsibility.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment to our office at the time of your appointment. If necessary, our billing manager will work with patients who require a payment plan.

If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call Inquiries regarding your coverage and benefits should be directed to your employer or insurance company.

Medical records for patients of Timothy J. Allen, MD may be obtained as follows:

Please allow up to 30 days for processing.

Fees for personal copies of your medical records are as follows:
▪Ten (10) or fewer pages: $18.53
▪Pages 11-40: $.85/page
▪Pages 41 and beyond: $0.57/page

There is no charge for medical records sent directly to another physician.
